The landing page lists a $250 tier at .80 EF and a $350 tier at .92 EF. The currently linked application lists the .80 EF/$250 tier but does not list the higher one. Carry the disagreement into the model check
Prepare this record before relying on a quoted rebate:

The linked form states a 60-day submission period from purchase and asks for the dated invoice, equipment details and installation information. It excludes warranty-replacement equipment and says funds and qualifications can change.
