Choose the problem the form actually offers

The container form lists damaged lids, missing after garbage pickup, damaged wheels, an additional can, cracked/split containers and Other. It asks for contact and service-address information, account/customer number, container count and comments. There is no separate missed-garbage-pickup option. Actual form

Use the comments to say what is wrong with the cart and which container is affected. For a pickup complaint, give Sanitation the service address, expected collection day, when the cart was placed out and what remained. These are LSL preparation suggestions, not a claim that a complaint was accepted or a crew caused damage.

If safe photographs help explain the condition, keep them for a follow-up request. The inspected container form has no photo-upload field; do not assume an attachment was transmitted with it. Submit personal account and contact details only through the City’s own route, not to LSL.

The published wait is not a completed work order

The form asks residents to allow one to two days for repair or delivery and directs questions to 318-329-2209, option 2. That is the City’s stated guidance, not a turnaround LSL observed. If the problem remains, refer to the original request and describe the still-unresolved cart condition rather than submitting a claim that repair already occurred. Form instructions

The page also says these electronic communications do not constitute legally required notice to the City. Do not rely on the form alone to meet a separate legal-notice requirement.

Do not assume every replacement or additional cart is free

Monroe’s 2026 refuse ordinance distinguishes City replacement for normal wear from loss, theft or misuse-related replacement costs. It also charges both standard residential service and rental per City-issued receptacle.
