Identify which site-review decision you need

The currently linked application distinguishes several situations. Use this comparison to discuss the proposal with the sanitarian:

Conditional approval is the important trap here: it does not mean an unsubmitted design has been approved. The form calls for the actual system and house plans in a further review before permit issuance. Keep the design and installation application consistent

The installation/alteration application asks for the parcel and water supply, bedrooms, treatment components, designer and registered installer. A discharging system has a separate section for its NPDES coverage and discharge information. These are not blank spaces for an owner to estimate an engineered design. Installation application

Before submitting, reconcile three identifiers across the documents: the same parcel, the same proposed house/use, and the same system design revision. Ask the designer or sanitarian to resolve differences. Keep the issued decision with any approved stamped plan; a fee receipt is not that decision.

The form requires an approved design to accompany the application if it is not already on file. It also requires protection of the initial and replacement system areas from traffic and disturbance. Installation is not the last owner handoff

The application describes an installer-prepared as-built record for completed installation or alteration and a separate operation permit. It also identifies a service-contract requirement. Before the construction contract closes, ask who supplies those records, what continuing service applies and where the owner keeps the final system description. Installation terms

Documents and limits

LSL read the Board’s operative program pages and all three pages of both linked applications on September 20, 2026. File-upload dates do not establish when every requirement was adopted. The older County directory is retained only to explain the route change, not as the source of today’s form fields.
